Professional Services Disclaimer
Covenstead NW, LLC provides psychological services by a licensed psychologist in accordance with Oregon state laws and the ethical standards of the American Psychological Association (APA). Services may include individual psychotherapy, consultation, and psychological assessment. These services are not a substitute for emergency care, medical treatment, or legal consultation.
If you are in crisis or experiencing a medical or psychiatric emergency, please call 911 or go to the nearest emergency room. You can also contact the 24/7 Suicide & Crisis Lifeline at 988.
Licensure Information
Joan Pugh, PsyDĀ is a Licensed Psychologist in the State of Oregon, license #2345. PsychologicalĀ services are only available to individuals who are physically located in Oregon at the time of each session.
Confidentiality
Your privacy is protected under HIPAA and Oregon state law. Information shared in therapy is confidential and will not be released without your written consent, except in the following legally required situations:
- If there is a risk of serious harm to yourself or others
- If there is suspected abuse or neglect of a child, elder, or vulnerable adult
- If records are subpoenaed or required by court order
Client-Therapist Relationship
A therapeutic relationship begins only after a formal intake session and completion of required consent paperwork. Submitting an inquiry via this website or engaging in email correspondence does not constitute a therapeutic relationship.
Telehealth Services
Covenstead NW, LLC offers HIPAA-compliant telehealth services via secure video. By engaging in telehealth, you acknowledge and consent to the following:
- You must be physically located in Oregon at the time of your session.
- You are responsible for maintaining a private and distraction-free environment on your end.
- Technology may occasionally fail; backup options can be discussed in advance.
- Telehealth may not be appropriate for all clinical situations; referrals or in-person options may be discussed if needed.
By initiating telehealth, you provide informed consent to receive psychological services electronically.
Client Portal
A secure client portal is provided for scheduling, secure messaging, intake forms, and billing. By using the portal, you agree to:
- Use it for non-urgent communication only
- Not use messaging as a substitute for therapy
- Keep your login information private and secure
The portal is not monitored outside of business hours. If you are in crisis or need immediate support, please contact emergency services or a crisis line.
Appointments, Cancellations & Attendance
Sessions are typically scheduled on a weekly to bi-weekly basis to support continuity and progress. Monthly sessions are not generally recommended until a stable and consistent relationship has been established.
Please notify your therapist in advance of any travel or schedule changes that may affect consistency.
Cancellation Policy: 48 hours' notice is required for rescheduling or canceling. Late cancellations or no-shows will incur a fee of $95. Frequent cancellations may affect your ability to retain a regular appointment time.
Fees & Payment
Covenstead NW, LLC bills insurance as well as accepts private pay clients. Payment (incl copays) is due at the time of service.Ā
Clients who are uninsured or self-pay will receive a Good Faith Estimate of costs in accordance with the No Surprises Act.
Your Right to a Good Faith Estimate
Under the No Surprises Act, you are entitled to receive a written Good Faith Estimate for the expected cost of services:
- You will receive this estimate before your first appointment and upon request.
- If you receive a bill that is $400 or more above the estimate, you have the right to dispute the charges.
